REQUEST FOR PROPOSALS FOR

ADMINISTRATIVE CONSULTING SERVICES

The Town of Farmerville has been approved for a three(3) grants under the Louisiana Watershed Initiative (LWI) – Round 1 for the purpose of 1.  Hwy 33 Ditch Hardening and Drainage Improvements, 2. James Addition Drainage Improvements, 3. Gulley Street Drainage Improvements. The Town of Farmerville is interested in procuring the services of an administrative consulting firm to administer and implement the project.

The procedures for the selection of this firm will be in accordance with the procurement requirements of the Louisiana CDBG Program.  All responses received will be evaluated in accordance with the selection criteria and corresponding point system that is identified in the request for proposals package.  That package also identifies the scope of services to be performed by the selected firm. The Town will award the contract to the respondent obtaining the highest score in the evaluation process.                           

Interested parties are invited to secure a proposal package from Farmerville Town Hall, 1024 Sterlington Hwy, Farmerville, LA 71241.  The response to this request must be received at the above-named address in such a manner that it is received no later than 12:00 PM on December 28, 2023.   Please state “Town of Farmerville Watershed Grants – RFP for Administrative Consulting Services” on the cover. Sealed bids for the following project will be received by the Town of Farmerville, Louisiana, at the office of Riley Company of Louisiana, Inc., 112 East Mississippi Avenue, Ruston, Louisiana 71270, either by mail or hand delivery, no later than 10:00 A.M. local time on FRIDAY, NOVEMBER 17, 2023  at which time all bids received will be publicly opened and read aloud.  Bids received after the 10:00 A.M. deadline will be returned via certified mail to the bidder unopened.    

PROJECT: Lift Station #10 Improvements

            PROJECT CLASSIFICATION: Municipal and Public Works

            CONTRACT TIME:  180 Calendar Days

            LIQUIDATED DAMAGES: $400.00 per Calendar Day

Union Parish Buildings and Ground Committee

Meeting Minutes

Monday, September 18, 2023

10:00a.m.

The Buildings and Grounds Committee was held on Monday, September 18, 2023, at 10:00a.m. at the Union Parish School Board. The meeting was called to order by Mr. Tommy Bennett. A quorum was established by roll call. Committee members in attendance were Tommy Bennett and Judy Mabry. Presentations were made by Greg Wallace and Tony Littleton on the CTE Building and Stadium Seating projects for Union Parish High School. CTE plans have been submitted to the state fire marshal’s office. The Bid Schedule has been created and provided to the superintendent’s office for processing. Bid advertisements will run for three consecutive weeks in the Banner, The Gazette, and the Ouachita Citizen beginning September 27, 2023. There will be a mandatory Pre-Bid Meeting for all companies who plan to bid on the project on October 17, and bids will be opened on October 26, 2023. The architects asked if a special meeting could be called to approve the bids immediately after that date, so the project could move forward. Mrs. Fine will reach out to Mr. Katz to determine if that is possible. The stadium bleacher drawings are complete except for a couple of specs. The bid process will begin next month. Mr. Robby Lindsay gave an update on the river lot properties and land swaps. Two parties have signed agreements and the appraiser has been contacted. No action was taken to approve a proposal by Matt Fife to allow river lot tenants to purchase land from him to swap with UPSB. Second discussion of proposed use of River Lot #50. Mr. Lindsay is to get estimated prices for renovation costs. No action was taken to reserve Lot # 50 for use by UPSD employees. No action was taken to sell abandoned camper at the river lots. Mrs. Fine will reach out to Mr. Katz about the legal process of donating verses bidding. The committee discussed the need for a new maintenance truck. Mrs. Fine notified them that a Capital Improvements Approval form has been submitted to use ESSER 3 funding, but we have not been notified as of Monday. The general fund could be used if it is not approved. The next Buildings and Grounds Committee will be scheduled as needed.  The meeting was adjourned.


REGULAR MEETING, UNION PARISH SCHOOL BOARD

October 16, 2023

The Union Parish School Board met in Regular Session at the Union Parish School Board Office in Farmerville, Louisiana, on Monday, October 16, 2023, at 6:00 p.m. 

President Dixson called the meeting to order and Mr. Reeves gave the invocation.

President Dixson called for a roll call to establish a quorum.  Board members attending were as follows:

          Mr. Gary Jackson, Principal of Union Parish Elementary School, and Nicholas McConnell, Principal of Union Parish High School, introduced to the Board students who scored Mastery and/or Advanced on the Spring 2023 LEAP 2025 assessment tests.

          The Union Parish Leadership Team presented to the Board their progress on the Union Parish Strategic Plan.

          On motion by Mrs. Neal, seconded by Mr. Reeves, the Board approved the 2023-2024 Pupil Progression Plan.

          On motion by Ms. Collins, seconded by Mrs. Stewart, the Board approved the Agreement to Exchange Land with Dwight and Gay Viereck.  Union Parish School Board will transfer Lot #10, bearing 911 address of 391 School Board Road, Haile, Louisiana.  Exchanger shall transfer to the Union Parish School Board a parcel of land of comparable value to the lot stated above.

          On motion by Mr. Reeves, seconded by Mrs. Neal, the Board approved the addendum to Renewed Contract and Act of Lease of Surface Rights agreement with Clifton Snell for Lot #5, at 355 School Board Road, Marion, Louisiana 71260, which adds Heath Snell as an additional tenant and makes no changes, and authorized the Superintendent to execute the said addendum.

          On motion by Ms. Collins, seconded by Mr. Reeves, the Board approved employing an appraiser for the following School Board lots located in Haile, Louisiana on School Board Road that in the future will be advertised for a land swap: Lots 3, 6, 9, 15, 18, 23, 24, 28, 34, 35, 38, 49, 53, and 55.

          On motion by Mrs. Mabry, seconded by Mr. Reeves, the Board granted permission for Donna Kay Darnell, Union Parish High School Louisiana Senior Beta State Sponsor to travel to the Beta State Conventions of Tennessee in Nashville, November 15-19, 2023 and of Arkansas in Hot Springs, Arkansas, January 17-19, 2024 as the Louisiana State Sponsor.

          On motion by Mrs. Mabry, seconded by Ms. Collins, the Board granted permission for Dr. Dennis Hensley, Jr., Band Director at Union Parish High School, to travel to San Antonio, Texas, February 7-10, 2024 to attend the Texas Music Education National Conference.

          On motion by Mrs. Mabry, seconded by Mrs. Neal, the Board approved the following policies at their second viewing:

The Board reviewed the Personnel Report.

          Superintendent Fine reported to the Board on Homecoming Week and invited Board Members to ride on the Central Office float.

          Fred Carr introduced to the Board the Concerned Citizens of Union Committee and read their mission statement.  He and Janet Gatson questioned the high number of school suspensions due to fighting.

          Marsha Andrews spoke to the Board about school performance scores and paraprofessionals.

          Superintendent Fine addressed all their concerns.

The Board, having set its next Regular Board Meeting for Monday, November 13, 2023, to be held at 6:00 p.m. at the Union Parish School Board Office in Farmerville, Louisiana, adjourned on this the 16th day of October, 2023.

Union General Hospital and the Union General Rural Health Clinic do not accept Aetna Medicare Advantage, Healthy Blue Medicare Advantage (Anthem B/C), United Medicare Advantage, or WellCare Medicare Advantage insurance for elective services. We will ALWAYS provide treatment for Emergency Services (in our Emergency Room), regardless of insurance coverage or ability to pay.

We were unable to negotiate contracts with these companies, and the rates that they pay are lower than our costs to provide services. For our Medicare patients, to use Union General Hospital or Union General Rural Health Clinic for elective services, you’ll need to have coverage under Traditional Medicare or through one of the company’s plans listed below.

Attention : All owners of Community Memorial Garden Cemetery Spaces Please note the cemetery maintenance will not be conducted on graves with rocks, lights and other memorabilia decorations around graves. Lawn equipment cannot work in this environment and it is very dangerous to operate to remove weeds. Since no funds are paid to maintain this cemetery, the maintenance has been voluntary. Effective September 12th all items including memorabilia, glass objects, plant hangers and permanent plantings shall not be removed from grave sites. All bids must be accompanied by bid security equal to five percent (5%) of the sum of the base bid and all alternates, and must be in the form of a certified check, cashier’s check or Bid Bond Form written by a surety company licensed to do business in Louisiana, signed by the surety’s agency or attorney-in-fact.  Surety must be listed on the current U.S. Department of the Treasury Financial Management Service list of approved bonding companies as approved for an amount equal to or greater than the amount for which it obligates itself in the Bond, or must be a Louisiana domiciled insurance company with at least an A- rating in the latest printing of the A.M. Best’s Key Rating Guide.  If surety qualifies by virtue of its Best’s listing, the amount of the Bond may not exceed ten percent of policyholders’ surplus as shown in the latest A.M. Best’s Key Rating Guide.  The Bid Bond shall be in favor of Union General Hospital, and shall be accompanied by appropriate power of attorney.  No Bid Bond indicating an obligation of less than five percent (5%) by any method is acceptable. 

The successful Bidder shall be required to furnish a Performance and Payment Bond written by a company licensed to do business in Louisiana, in an amount equal to 100% of the Contract amount.  Surety must be listed currently on the U.S. Department of Treasury Financial management Service List (Treasury List) as approved for an amount equal to or greater than the contract amount, or must be an insurance company domiciled in Louisiana or owned by Louisiana residents.  If surety is qualified other than by listing on the Treasury list, the contract amount may not exceed fifteen percent of policyholders’ surplus as shown by surety’s most recent financial statements filed with the Louisiana Department of Insurance and may not exceed the amount of $500,000.  However, a Louisiana domiciled insurance company with at least an A- rating in the latest printing of the A.M. Best’s Key Rating Guide shall not be subject to the $500,000 limitation, provided that the contract amount does not exceed ten percent of policyholders’ surplus as shown in the latest A.M. Best’s Key Rating Guide nor fifteen percent of policyholder’s surplus as shown by surety’s most recent financial statements filed with the Louisiana Department of Insurance.  The Bond shall be signed by the surety’s agent or attorney-in-fact. 

Bids shall be accepted from Contractors who are licensed under LA. R.S. 37:2150-2163 for the classification of Building Construction.  Because the party providing funding for the project requires compliance with state bid laws, bidder is required to comply with provisions and requirements of LA R.S. 38:2212 unless waived.  No bid may be withdrawn for a period of thirty (30) days after receipt of bids, except under the provisions of LA. R.S. 38:2214. 

Attention is called to the fact that the Contractor must ensure that employees and applicants for employment are not discriminated against because of race, creed, color or national origin, and must follow certain contracting guidelines established by the United States Department of Agriculture.  The contractor must comply with the minimum rates for wages for laborers and mechanics as determined by the Secretary of Labor in accordance with the provisions of the Davis-Bacon and Related Acts.

The Owner reserves the right to reject any and all bids for just cause.  In accordance with La. R.S. 38:2212 (A)(1)(b), the provisions and requirements of this Section, those stated in the advertisement for bids, and those required on the bid form shall not be considered as informalities.

Committee Meeting Minutes

Tuesday, July 25, 2023

10:00a.m.

The Buildings and Grounds Committee was held on Tuesday, July 25, 2023, at 10:00a.m. at the Union Parish School Board. The meeting was called to order by Robby Lindsay. A quorum was established by roll call. Committee members in attendance were Sharon Dixson, Judy Mabry, and Sharon Stewart. Tommy Bennett was absent.

Greg Wallace and Tony Littleton with Wallace Design first gave an update on the stadium bleacher project. The main project will add 225 additional seats to the present stadium. A survey has been conducted and Matt Massey will send the CAD files that has the specific dimensions, foundation details, etc. The additional end-zone seating will be listed as an alternative project. ESSER Funding of $300,000 will cover the entity of the project. has The project will have process of the career technical building. On motion by Mrs. Mabry and second by Mrs. Stewart the committee decided to place it on the Board agenda for approval.

Greg Wallace and Tony Littleton with Wallace Design also gave an update on the career technical building project. Well on their way with the project. Preliminary drawings were shown. Mechanical engineering, plumbing, and other entities have reviewed the drawings and added information needed. The new requirements and codes have been added. Plans will be finalized in a couple of weeks and sent to the Fire Marshal’s office. Plans are in the superintendent’s office for viewing.

Matt Fife and his wife were present. They have land on Turkey Creek Road that they would like to swap for some river lots. An appraisal will have to be conducted to determine a fair trade, but he is willing to go up to 20 lots. On motions by Mrs. Stewart and second by Mrs. Mabry the land swap will be added to the August Board agenda.

Mr. Lindsay shared with the committee that Lot #50 had been surrendered back to Union Parish School Board because of health issues of the lease. The lot has an extremely nice camp on it, so he brought the suggestion to Sup Fine about retaining it for school board employee use. He explained that Mr. Katz confirmed it could be used in that manner if the Board approved. Specifics about rental, etc. will be determined once the Board approved. On motion by Mrs. Stewart and second by Mrs. Dixson the proposition will be placed on the August agenda.

The meeting was adjourned by motion from Mrs. Stewart and second by Mrs. Dixson.
